Appointment Process

Easy Steps To Begin Your Care Journey

Starting your health journey with us is straightforward and stress-free.. Follow these simple steps to schedule your appointment and prepare for your visit with our dedicated specialists:

  • Call Our Office or Use Our Patient Portal

    For new patients, call us to find a convenient appointment time or use our online Patient Portal to request an appointment. Our team is here to assist with any preliminary questions, including those about new patient forms.

  • Fill Out Your Forms

    Complete your new patient and medical information disclosure forms. To expedite your check-in, you can fill out these forms online via our Patient Portal before your visit.

  • Meet Your Doctor

    Bring your insurance card and arrive 10-15 minutes early to your appointment. Remember to call us if you need to cancel or reschedule. We may request a urine specimen, so please plan accordingly.

  • Follow Up

    For any billing or insurance questions after your visit, contact our office manager, Mary Howell. Use the online patient portal for non-emergent questions (response time 24-48 hours) and call us for urgent needs. Access your PAP results on LabCorp's link from our page or wait for our call/email for any results.

  • Future Visits

    We’ll guide you on when to schedule your next visit, typically recommending annual exams for ongoing health management. We'll advise on any further referrals or tests needed at the end of your appointment.
