Please bring your photo identification and insurance cards to your first visit with our team. Depending on your insurance coverage, you may be asked to pay for a portion of your bill and/or a copay at the time of your service for each visit.
Yes, a copay is expected at the time of service for each visit.
As a courtesy to our patients, we bill the insurance company for your visit. If your insurance company pays for a service that you have already paid for, we will credit your account or, if preferred, we will reimburse you.
Please promptly notify our team of those changes and provide updated copies of your insurance cards. Your attention to this detail will streamline the billing process for your care.
Starting your health journey with us is straightforward and stress-free.. Follow these simple steps to schedule your appointment and prepare for your visit with our dedicated specialists:
For new patients, call us to find a convenient appointment time or use our online Patient Portal to request an appointment. Our team is here to assist with any preliminary questions, including those about new patient forms.
Complete your new patient and medical information disclosure forms. To expedite your check-in, you can fill out these forms online via our Patient Portal before your visit.
Bring your insurance card and arrive 10-15 minutes early to your appointment. Remember to call us if you need to cancel or reschedule. We may request a urine specimen, so please plan accordingly.
For any billing or insurance questions after your visit, contact our office manager, Mary Howell. Use the online patient portal for non-emergent questions (response time 24-48 hours) and call us for urgent needs. Access your PAP results on LabCorp's link from our page or wait for our call/email for any results.
We’ll guide you on when to schedule your next visit, typically recommending annual exams for ongoing health management. We'll advise on any further referrals or tests needed at the end of your appointment.
We provide comprehensive healthcare for women, by women.
